[] I confirm I have enclosed 2 letters of support.
[] I confirm I have enclosed a one-page narrative statement of my achievements.
[] I confirm that I have a Master’s degree in Library and Information Science and /or School Library Media Specialist completed before November 30 of this year. (32 hours of Library Science, Type 10 Certification. Indicate all specific certifications held. A Teaching Certificate Endorsement is not applicable)
[] I confirm that I am employed with an Illinois library/School Media Center.
[] I commit to attending all three leadership sessions and to participate in intersession activities. I understand that if I am unable to attend any one of the sessions that I am automatically eliminated from the Synergy: The Illinois Library Leadership Initiative, because each Synergy session builds upon the leadership skills of the previous sessions.
[] I agree to assume responsibility for all travel and for personal and incidental expenses.
[] I agree to notify the Shawnee Library System immediately if I am selected but am unable to attend all sessions. I understand that Synergy is a highly competitive leadership training initiative and only 30 applicants are selected from many applications each year. My early notification may allow another applicant to attend.
